This application is the first step in determining residency eligibility at Her House of Grace. It allows us to learn more about each applicant, review eligibility, and determine whether our shared-living environment is the right fit.

Applicants may apply independently or be referred by healthcare providers, community organizations, veterans' programs, government agencies, or other approved referral partners.

To begin the review process, please complete the application and upload the requested documentation. Additional information may be requested depending on the applicant's funding source or referral. 

A $50.00 non-refundable application fee is required for private-pay applicants to begin the application review process. Payment may be submitted via Zelle. To ensure your payment is properly matched with your application, please include the following in the memo line:

First and Last Name – Application Fee – Month/Year

Example: Jane Doe – Application Fee – January 2026

If you have any questions regarding your application or payment, please contact Her House of Grace before submitting your application.

Application fees for agency, government, or third-party referrals will be handled in accordance with the applicable referral or funding process.

Our team reviews each application individually and will contact applicants or referring agencies regarding the next steps within 48hrs.

Background Review Authorization:

By submitting this application, you authorize Her House of Grace to conduct a background review as part of the residency evaluation process. Background information is reviewed on an individual basis and is not automatically disqualifying.
